SpikeFest Registration Form

In order to register your church/fellowship/other teams for the 2008 SpikeFest, please complete the following information below. We require a FULL payment and roster at the time of registration. Please mail the registration to New Covenant Fellowship Church (checks payable to NCFC). DO NOT SEND CASH.

Early registration – On or before April 23, 2008 ($150 per team) -postmarked on envelope
Regular registration – On or before May 12, 2008 ($175 per team) -postmarked on envelope
*Guaranteed spots for first 16 teams.

Your effective date of registration (the date we use when considering early registration) is the postmarked date of your FULL payment. An e-mail will be sent to confirm your registration.
